Living Will: A living will is a document that lets other individuals know what someone’s wishes are for their end of life medical care in case the individual is no longer able to communicate their wishes. Durable Power of Attorney for Health Care: A durable power of attorney for health care is a document that designates a person to make a medical decision on one’s behalf if the designator is unable to communicate their medical wishes. Do Not Resuscitate (DNR) Order: A DNR order alerts medical professionals that the individual does not want to be resuscitated in case of a medical emergency. Physicians Orders for Life Sustaining Treatment (POLST) form: A POLST form allows an individual to provide more details about the type of care they wish to receive in a medical emergency if they are unable to communicate. For example, the form can let one choose whether or not they want antibiotics, to be intubated or have a feeding tube.
