Serving Suffolk County, Long Island, New York since 1970, Swan Lake Rehab has grown into a magnificent 120-bed facility. Since its founding, its mission has been to provide clients with the highest level of physical and occupational therapy, skilled nursing care and rehabilitative services.

Eric Fischbein
Eric Fischbein

Administrator

With his signature personal touch, Eric brings many years of administration experience to Swan Lake Nursing & Rehabilitation. In September 2022, Eric joined the Swan team as Facility Administrator with a strong focus on quality measures to ensure residents receive the highest level of care. Eric holds a degree in Business Administration and Management from Brooklyn College.

Patricia Wyble
Patricia Wyble

Director of Nursing

Trish has been a nurse for over 28 years. She has served in the military where she first learned about leadership and respect as well worked for the NYS Department of Health. She has valuable experience as a Registered Nurse in Long-Term care and is energetic, personable and interfaces well with patients and families. Patricia has a genuine desire to improve the care and quality of life of her patients. She is truly an advocate for patients' rights, sensitive to patients’ needs and concerns. She takes her years of experience and energy and applies it to her leadership role ensuring competent and dignified care.

Kimberly Rego
Kimberly Rego

Director of Rehabilitation

Kimberly Rego M.S. CCC-SLP is a national certified and state licensed Speech-Language Pathologist. Kimberly is a Magna Cum Laude graduate of Ithaca College, where she received her B.S. and M.S. in Speech-Language Pathology and a Minor in Spanish. Kimberly has over 20 years of experience as an SLP. During her professional career she has been fortunate to work in a variety of settings, including a Level 2 trauma hospital, Skilled Nursing Facilities, VA facilities and Home Health. She has worked with a wide range of populations from infants to geriatrics and disorders. Kimberly is licensed by the board of New York State and has her Certificate of Clinical Competence from the American Speech Language Hearing Association. Kimberly currently resides on Long Island with her family.

Mary Desmond
Mary Desmond

Director of Admissions

Mary graduated with a Bachelor's degree in Communications and Psychology from SUNY Oneonta. She has worked with the geriatric population for twelve years and has assumed many roles in long-term care, including caregiving, marketing and admissions. Mary joined Swan Lake Rehabilitation in 2011 and has demonstrated her commitment to our mission of providing the highest quality of care and personalized customer service for our guests.

Jennifer Healey
Jennifer Healey

Director of Recreation

Jennifer loves bringing smiles to the residents' faces as she provides meaningful and purposeful activities every day. Jennifer has a Bachelor's degree in leisure service management and has worked in the healthcare field for nine years. She began her career in the long-term care setting as a recreation therapist and was promoted to Director. Jennifer wanted a career where she could help others and make an impact on their lives, which is what led her to the path of therapeutic recreation. In her spare time, she enjoys music, crafts, travel and spending time with her friends and family.

David Morales
David Morales

Director of Maintenance

David is a skilled operations management professional with over 28 years of experience overseeing Environmental Services. Having previously managed large scale properties, he plans and executes complex maintenance, renovation and new construction projects and has proven success overseeing multiple departments and facilities. He consistently evaluates processes and implements change to enhance strategic effectiveness across all levels of his departments.

Richie Kroebel
Richie Kroebel

Food Service Director

Richie started his culinary journey after graduating culinary school back in 2003. He began his career as a prep cook, working his way up to become a sous chef in a corporate dining setting. After 16 plus years of corporate cooking, Richie shifted gears and in 2019 entered the Health Care industry. As a Food Service Director, he found his passion for making our residents happy with the power of food. Richie joined our team in November of 2021. With his passion for cooking, and strong leadership skills, Richie has worked with the dietary department to revamp the menus to provide delicious, nutritious meals for our residents. He loves spending time giving back to his community and is a big Foodie!

Charmine Roseau
Charmine Roseau

Director of Social Work

Charmine is a graduate of Adelphi University prestigious Social Work program and an undergraduate of Brooklyn College in Health Nutrition Sciences. Coming to Swan Lake Nursing and Rehabilitation with over 10 plus years of experience, her main focus is to advocate for the residents. At Swan Lake, our team's goal is to enhance the overall well-being and help meet basic and complex needs of the residents. Charmine has a background working with many different populations and types of people, particularly focused on geriatrics. She has lived in the United States for 34 years, originally from Haiti and is fluent in French/Creole.

Registered Dietitian

Skylar is a recent graduate of Stony Brook University with a Masters of Science in Nutrition. She aims to deliver quality care to all residents by prescribing food as a natural medicine for their specific nutritional needs. Her approach is to treat everyone as unique individuals in order to build lifestyle skills and support resident-driven goals.
